<p>Once clear margins were obtained, the 3.8 cm defect was closed using an advancement flap.  The patient tolerated the procedure without any complications.</p>

<p>Mohs micrographic surgery is a procedure whereby skin cancers are excised, and the tissue is stained with hematoxylin and eosin and examined using horizontally-oriented frozen section technique with detailed mapping of the surgical specimens.  If there is any residual tumor, the process is repeated until the margins are free of tumor.  Indications for this procedure includes skin cancers of large size greater than two centimeters, location on areas that are cosmetically sensitive and require tissue-sparing techniques, tumors with poorly defined borders, tumors with aggressive histology or in areas more likely to metastasize, such as the periorbital, nasal, perioral, and auricular region, as well as recurrent tumors.<br />
This is a perfect example of a lesion that extended beyond the visible margins, and the final defect exceeded any expectations.<br />
Mohs surgery is a very useful procedure that will help us in these types of cases and is a very simple technique that when used in the proper setting with trained staff will run without many difficulties.</p>
